Red Star's Milan Bisevac was sent off in the 29th minute for a foul on DaMarcus Beasley when he was through on goal.
Having lost
3-2 in Belgrade a fortnight ago, PSV took the aggregate lead after 32 minutes thanks to Mark van Bommel, with a penalty, and Beasley.
Van Bommel, John de Jong and Jan Vennegoor of Hesselink completed the rout in the second half.
PSV got the desired early goal in the ninth minute when skipper Van Bommel scored from the spot after Aleksandar Lukovic tackled Ji-Sung Park from behind.
Bisevac's dismissal meant that qualification for the Champions League became almost mission impossible for Red Star and three minutes later Beasley scored PSV's second from a Park cross.
Van Bommel added his second goal on 56 minutes, picking up a defence splitting pass from Ooijer. De Jong made it 4-0 two minutes later and Vennegoor of Hesselink headed home with 10 minutes to play.
