The 38-year-old Kosgei finished in 2 hours, 10 minutes, 54 seconds to nip the course record by one minute and 16 seconds.
Halina Karnatsevich of Belarus won the women's race in 2:34:35.
Iaroslav Mushinski of Moldova won his fifth career Belgrade title in men's half-marathon, finishing in 1 hour, 4 minutes, 51 seconds.
Former track and field superstar, Carl Lewis fired the starting signal, and presented the awards afterwards, including the Fred Lebow Trophy for both winners. The 44-year-old Lewis was in Belgrade to promote the race.
