Spain goalkeeper Iker Casillas misjudged a deep Goran Gavrancic free-kick which left Dejan Stankovic with a clean shot on goal. His effort was deflected in off Kezman to make it 1-1.
Guests played out seven minutes of time added on with 10 men after defender Igor Duljaj received his second yellow of the night.
The result means Serbia-Montenegro remain top of the group with 16 points, two ahead of Spain with both teams having two games left to play.
