The Celta Vigo striker fired a left-footed shot into the net from a Danko Lazovic pass just six minutes into the match on Sunday, and from then on the writing was on the wall for Slovakia.
Milosevic was replaced after 60 minutes by Belgium's Anderlecht striker Nenad Jestrovic and just when it seemed the match was destined to finish 1-0, Serbia and Montenegro struck two minutes into injury time to press home their advantage.
Midfielder Jovan Markoski, who replaced Milos Maric in the 80th minute, intercepted a Slovakian pass and sent the ball to Jestrovic, who dribbled into the area and scored with a right-footed shot.
Slovakia ended the round robin firmly at the foot of the table with two defeats after losing to hosts Japan 3-1 on the opening day Friday in Hiroshima.
Serbia and Montenegro will take on Japan Tuesday in the final at the Yokohama stadium, the site of the 2002 World Cup final between Brazil and Germany, vying for a top prize of 100,000 dollars.
